OSHA runs two crane inspection clocks. In general industry, 29 CFR 1910.179 requires daily visual checks of operating mechanisms, hooks, and hoist chains, monthly signed certification records for hooks, chains, and running ropes, and a complete "periodic" inspection every 1 to 12 months depending on service. On construction sites, 29 CFR 1926.1412 requires a competent-person visual inspection each shift, a documented monthly inspection, and a documented annual/comprehensive inspection by a qualified person — with 1926.1413 putting wire rope on the same shift/monthly/annual cadence. The two regimes also split on paperwork: 1910.179 requires surprisingly few records, while construction inspections come with retention periods OSHA can ask for.
Which OSHA rule covers your crane
29 CFR 1910.179 applies to overhead and gantry cranes in general industry — including semigantry, cantilever gantry, wall cranes, storage bridge cranes, and others with the same fundamental characteristics. Per 1910.179(b)(1), these are grouped because they all have trolleys and similar travel characteristics.
That "trolleys and similar travel characteristics" language is also why some equipment is out. OSHA's March 4, 1991 interpretation letter confirms 1910.179 does not apply to underhung cranes, overhead hoists, or monorails, pointing at the time to the ANSI B30.11 consensus standard for underhung cranes and monorails — a volume since consolidated into ASME B30.17, which is the current standard for that equipment. Excluded doesn't mean unregulated: OSHA's February 12, 1991 letter states that under Section 5(a)(1) of the OSH Act — the general duty clause — OSHA's policy is that employers must comply with safety standards recognized in the industry, even where OSHA has no standard of its own. A hoist that's part of a covered crane, though, is inspected as part of that crane — its chains, ropes, and limit switches are all called out in 1910.179's inspection paragraphs.
Cranes and derricks used in construction work fall under 1926 Subpart CC, where 1926.1412 sets the equipment inspection cadence and 1926.1413 covers wire rope.
How often do cranes need to be inspected: 1910.179's two classes
For cranes in regular service, 1910.179(j)(1)(ii) divides inspections into two classifications by interval, with the interval depending on the nature of the critical components and their exposure to wear, deterioration, or malfunction:

Frequent inspection (1910.179(j)(2)) covers, at daily intervals: all functional operating mechanisms for maladjustment interfering with proper operation, and air or hydraulic systems — lines, tanks, valves, drain pumps — for deterioration or leakage. Hooks get a daily visual check for deformation or cracks, and hoist chains (including end connections) a daily visual check for excessive wear, twist, distorted links, or stretch beyond the manufacturer's recommendations. At frequent intervals, operating mechanisms are also checked for excessive wear of components and rope reeving for compliance with the manufacturer's recommendations. The paragraph also requires observation during operation for defects appearing between regular inspections — the operator who notices jerky travel or an odd noise is part of the inspection program. Separately, 1910.179(n)(4)(i) requires the upper limit switch of each hoist to be tried out under no load at the beginning of each operator's shift.
Hook criteria are specific: hooks with cracks, throat opening spread more than 15 percent beyond normal, or twist of more than 10 degrees from the plane of the unbent hook are referred to 1910.179(l)(3)(iii)(a) — in practice, discard. Weld or reshape repairs are "not generally recommended," and if attempted, the hook must pass a (k)(2) load test before reuse.
Periodic inspection (1910.179(j)(3)) is a complete inspection of the crane at 1-to-12-month intervals. It includes all the frequent items plus: deformed, cracked, or corroded members; loose bolts or rivets; cracked or worn sheaves and drums; worn, cracked, or distorted parts such as pins, bearings, shafts, gears, rollers, and locking and clamping devices; excessive wear on brake system parts, linings, pawls, and ratchets; load, wind, and other indicators over their full range; powerplants; chain drive sprockets and chain stretch; and electrical apparatus for pitting or deterioration of controller contactors, limit switches, and pushbutton stations. OSHA's August 28, 2003 interpretation letter treats "complete" as a performance requirement: if defects are detectable through visual inspection, visual meets the intent of the standard; if not, disassembly of some components may be necessary.
Note what 1910.179 does not say: the word "annual" never appears for in-service cranes. The 12-month outer bound of the periodic band is where the industry's annual-inspection habit comes from — a defensible, conservative reading, and the cadence most plants and inspection providers run.
Running ropes (1910.179(m)(1)) get a thorough inspection at least once a month, with a certification record — date, inspector's signature, rope identifier — kept on file where readily available to appointed personnel. Conditions to watch: reduced diameter, broken or worn outside wires, corroded or broken wires at end connections, defective end connections, and severe kinking, crushing, cutting, or unstranding. Rope idle a month or more (crane shutdown or storage) must be thoroughly inspected before use by an appointed person, whose approval is required for further use — again with a certification record.
Construction cranes: shift, monthly, and annual (1926.1412 and 1926.1413)
Subpart CC is more prescriptive about cadence and paperwork:

Shift inspection. A competent person must begin a visual inspection prior to each shift the equipment will be used, completed before or during that shift — observation for apparent deficiencies across 14 listed items, including control mechanisms, pressurized lines, hydraulic fluid level, hooks and latches, wire rope reeving, the wire rope itself per 1926.1413(a), electrical apparatus, tires, ground conditions, level position, cab windows, rails, and safety devices and operational aids. No documentation is required — but any deficiency that's a safety hazard takes the equipment out of service until corrected.
Monthly inspection. Each month the equipment is in service, it must be inspected at shift-inspection scope — and this one is documented: the items checked and results, plus the inspector's name, signature, and date, retained by the employer that conducts the inspection for at least three months.
Annual/comprehensive inspection. At least every 12 months, a qualified person must inspect the equipment, with disassembly as necessary, across 21 listed categories — structure, boom and jib, sheaves and drums, pins, bearings, shafts and gears, brake and clutch parts, safety devices and operational aids, power plants, hydraulic and pneumatic hoses, pumps, valves and cylinders, outrigger pads, electrical components, labels, and the operator's seat — including functional testing. Documentation is retained for a minimum of 12 months.
Event-triggered inspections sit on top of the calendar: a qualified-person inspection with functional testing after modifications or repairs and adjustments relating to safe operation, a post-assembly inspection to confirm configuration against manufacturer criteria, a stop-and-inspect after severe service such as a possible overload or shock loading, and a monthly-level qualified-person inspection before returning equipment idle three months or more to use. Two more provisions matter for anyone running an inspection program: manufacturer inspection procedures that are more comprehensive or more frequent than the section's requirements must be followed (1926.1412(j)), and all inspection documents must remain available to everyone who conducts inspections under the section for the full retention period (1926.1412(k)).
Wire rope (1926.1413) has its own deficiency taxonomy. The shift inspection looks for Category I deficiencies (significant distortion, corrosion, electrical arc or heat damage, improperly applied or damaged end connections), Category II (broken-wire counts — for running ropes, six randomly distributed broken wires in one rope lay or three broken wires in one strand in one rope lay — or diameter reduction of more than 5% from nominal), and Category III (core failure in rotation-resistant rope, prior electrical contact with a power line, a broken strand). The monthly rope inspection is documented with three-month retention; the annual covers the entire rope length, by a qualified person, documented with 12-month retention — with a feasibility exception allowing up to 6 additional months for running ropes.
The records OSHA actually requires
Here's the nuance that surprises most plant managers: 1910.179 requires no written record of the periodic inspection itself. The certification records the standard demands are for hooks (monthly), hoist chains (monthly), and running ropes (monthly) — each with the date, the inspector's signature, and the serial number or other identifier of the item inspected — plus rated load test reports on file. That's the complete list. Construction is the opposite: monthly and annual equipment and rope inspections must be documented, with three-month and 12-month retention respectively.
Nobody should run a periodic inspection without a record, though. ASME B30.2 — the consensus standard 1910.179 was built from — expects dated inspection records, and so will your customers' insurers and auditors. A signed report is also the only proof the inspection happened at all. One more point in software's favor: OSHA's October 21, 1997 interpretation letter accepts an electronic system in lieu of a physical signature on the monthly certification records, provided the system identifies the inspector and constitutes their certification that the record is true and complete. Electronic crane inspection records have been OSHA-acceptable since 1997.
Load testing: the 125 percent rule
Prior to initial use, all new and altered cranes must be operationally tested — hoisting and lowering, trolley travel, bridge travel, and limit switches, locking and safety devices — with hoist limit switch trip settings determined by test with an empty hook traveling at increasing speeds up to maximum (1910.179(k)(1)). For the rated load test, 1910.179(k)(2) sets the ceiling: test loads shall not be more than 125 percent of the rated load unless otherwise recommended by the manufacturer, and test reports must be kept on file where readily available to appointed personnel.
Is the load test actually mandatory? Yes. OSHA's August 28, 2009 interpretation letter settled a long-running ambiguity: the 1984 deletion of advisory language from (k)(2) did not eliminate the requirement — (k)(1)'s "tested to insure compliance with this section" still requires new and altered cranes to be load tested, and modified or rerated cranes must be tested under (b)(3), which invokes (k)(2).
Who may inspect: no third party required
1910.179 assigns duties to designated persons — defined in 1910.179(a)(35) as "selected or assigned by the employer or the employer's representative as being qualified to perform specific duties" — and to appointed persons for rope approvals. Subpart CC uses the 1926.1401 definitions: a competent person (capable of identifying existing and predictable hazards, with authorization to take prompt corrective measures) handles shift and monthly inspections; a qualified person (recognized degree, certificate, or professional standing, or extensive knowledge, training, and experience) handles the annual/comprehensive, post-assembly, post-repair, and post-modification inspections.
What neither standard requires is an outsider. OSHA's February 1, 1993 interpretation letter is blunt: cranes used exclusively in general industry or construction "are not required, under OSHA regulations, to be certificated by anyone," and the competent person "can be the equipment owner's maintenance personnel or any other person the owner chooses" — a disinterested third party is not required. OSHA requires competence, not independence. In practice, that competence bar is exactly why plants outsource: a qualified-person annual with disassembly, functional testing, and defensible documentation is a specialist's job, and hiring an inspection service is how most employers meet it. One caveat for multi-state operators: some OSHA state plans are stricter — California requires licensed certificating agencies for certain cranes — so check the plan where the crane lives.
Idle and standby cranes
1910.179(j)(4) scales the return-to-service inspection to how long the crane sat. Idle 1 to 6 months: a frequent-level inspection plus the (m)(2) rope inspection before service. Idle more than 6 months: a complete inspection — frequent and periodic items — plus the rope inspection. Standby cranes must be inspected at least semi-annually at frequent level plus rope. On construction sites, equipment idle 3 months or more gets a monthly-level inspection by a qualified person before initial use.
How this differs from ASME B30.2
1910.179 is old law with a frozen baseline: new overhead and gantry cranes constructed and installed on or after August 31, 1971 must meet ANSI B30.2.0-1967, the edition incorporated by reference. The section's text dates to 1974, and its most recent amendment (November 18, 2016) only updated ladder and handrail cross-references — the inspection paragraphs haven't changed. ASME B30.2, meanwhile, has kept revising, and it expects the dated inspection records that OSHA's periodic inspection never demands. OSHA itself won't interpret B30.2's internals — its 2003 letter defers that to the ASME B30 Committee.
